24.1 Health Plan Information
Employees of ADU receive healthcare benefits from the integrated delivery system of Florida Hospital which are subject to change from year to year. For questions regarding current healthcare benefits, reference Florida Hospital Employee Health Care Advantage and Network Directory. Limited family (one adult and one child) and family coverage are also available at an additional cost. There are two medical plans to choose from: the Health Savings Plan or the Traditional Plan.
24.2 HealthCare Plan for Employees
The employee healthcare plan provides medical coverage options for hospitalization and other major medical services at a minimal cost to full-time and eligible part-time employees. As a full-time employee, you are required to have this coverage unless you show satisfactory proof of coverage under another plan.
24.3 Dental Insurance
Dental insurance coverage is designed to help ease the financial burden of dental expenses. You may choose to cover yourself, your spouse and your dependents up to the end of the month of their 26th birthday. You can choose between Delta Dental PPO or Delta Care USA Managed Care.
24.4 Cancer and other Specified Diseases
The Cancer and Other Specified Diseases Plan is designed to pay cash benefits directly to you, in addition to any other medical plan you may have in the event that you or a covered family member is diagnosed with cancer or one of the 30 other specified diseases.
24.5 Short-Term Disability
Short-term disability coverage is offered to those who work 32 or more hours per pay period to provide monthly financial protection should you become ill or injured (non-work related) and unable to work. The benefit amount is 60% of your current weekly base income to a maximum of $5,000 per week and starts on the fiftenth day of an illness or on the first day after an accident. This benefit is provided to you at no cost; ADU pays the premium.
24.6 Long-Term Disability
Long-term disability coverage is offered to provide you with financial protection for more serious or longer term disabilities. Coverage under this plan is available to full-time employees only. Benefits begins after 180 days of disability and the amount is 60% of your monthly earnings with a maximum monthly benefit of $6,000.
24.7 Life Insurance
Basic Group Life and Accidental Death & Dismemberment (AD&D) insurance in the amount of your annual salary (plus an equal amount in the event of accidental death) is provided by your employer to all full-time employees. Eligible part-time employees may also participate for a minimal monthly cost. You may purchase additional insurance in an amount equal to one, two, three, or four times the amount of your annual salary. Dependent life insurance may also be purchased.
24.8 Vision Care
Vision Insurance is offered to eligible full-time and part-time employees and dependents through VSP.com.
24.9 Flexible Spending Accounts
If you have major out-of-pocket expenses covered by insurance, or child/elder care expenses, flexible spending accounts can help. A flexible spending account allows you to make these payments with your earned income before taxes are deducted. This saves you tax dollars. A Connect Your Care debit card is issued from which health care services are paid.
24.10 Retirement Plan
Florida Hospital maintains an excellent retirement plan for eligible full-time employees. The plan–called the Adventist Healthcare Retirement Plan (AHRP)–provides for employer cash contributions to employee accounts. Florida Hospital also matches a portion of the funds that employees voluntarily contribute to their AHRP account.
24.11 Employee Assistance Program
The Employee Assistance Program (EAP) is a free, confidential counseling program designed to help employees and members of their families resolve personal problems. When necessary, referrals are made to other professionals and appropriate agencies for counseling, legal services, and financial services. Your Financial Services benefit includes 30 days of unlimited telephonic assistance with the financial coaching staff. The EAP also offers online services with a full collection of information tools and resources to support emotional well-being, including hundreds of tip sheets, articles, and resource guides. 24.12 ADU Tuition Benefit
Full-time employees, and their immediate family members (spouse and children only) may take undergraduate classes at ADU with tuition and required (such as matriculation, lab, professional fees, etc.) fees waived. Full-time employees are eligible for tuition and required fees for MHA/EMHA courses at ADU with tuition and required matriculation and professional fee, if approved by administration. ADU employees are not eligible for the Florida Hospital Employee tuition benefits.
- Full-time Employees:
- Qualify for free undergraduate tuition and fees, either on campus or online.
- Qualify for free graduate MHA/EMHA tuition and fees either on campus or online if approved by administration via the Advanced Academic Study Application.
- A maximum of four hours may be taken during the workday, and hourly employees must clock out while in class.
- Approval: Before enrolling in a course the employee must have the permission of their Department Chair, Director or immediate supervisor and submit a completed “Tuition Benefit” form (see Appendix N ) each trimester to the Human Resources Office for approval.
- Immediate Family Members (spouse and children only):
- Qualify for free undergraduate tuition and fees, either on campus or online.
- Approval: The immediate family member must submit a completed “Tuition Benefit” form (see Appendix N ) each trimester to the Human Resources Office for approval.
- Fees: Books and elective fees (such as finance, parking, etc.) will be the responsibility of the faculty or family member.
- FAFSA/Aid Application: The full-time employee or the family member will be required (if they have not already earned a baccalaureate degree) to complete the FAFSA and the ADU Aid Application to determine eligibility for grants before the tuition benefits will be applied to the appropriate account.
24.13 Personal Information Access
Employees have access to their personal information online. Please visit https://public.floridahospital.org/HROnline/portal/signon to sign on. You may view your paycheck, change your address, update W-4, view PDO, and update direct deposit information.
24.14 Health Club Membership Reimbursement
Full-time employees are eligible to receive up to a $50 monthly reimbursement for gym memberships. In order to receive this benefit, employees need to use the membership a minimum of three times a week. Submit expense reports, with all necessary signatures, to the Office of Financial Services.
24.15 FH Rewards- Merchant Discounts
ADU employees are considered Florida Hospital employees, which entitles them to benefit from the discounts offered by merchants who are affiliated with Florida Hospital. Please visit https://ahs.perkspot.com/login for a complete listing.
24.16 Discounts on Event Tickets
ADU employees also get discounted tickets to theme parks, shows, and much more. Please visit www.ticketsatwork.com for a complete listing. You will need to register on the website. For first time users, please use FLHOSP as your password. Once you complete that step, you will be able to create a screen name and your own personalized password.
24.17 FH Credit Union
